If you are looking for more of a 3D design you can either model with progeCAD ACIS solids or use a third party ACIS compatible CAD like Alibre then import the ACIS solid to progeCAD:

Does the ACIS version have elbows and other pipe fittings? Or would they have to be created?

progeCAD 3D is ACIS based, but doesn't offer parametrics and doesn't include any solid 3D blocks with the software.

Alibre includes a large quantity of 3D CAD symbols for piping and related things if that is really what you are looking for. They have also posted many things online:

Alibre offers a free version 3D CAD software called Xpress, which starts automatically after the 30-day full version trial. But assembly sizes are now limited to five unique parts (as of January 2009) in the free version, and there is no 2D layout drawing with the free Alibre Xpress 3D. But Xpress is a good start just to work with a free 3D design program to see if the full version meets your needs.

There are also a couple other sites with free 3D CAD symbols that work with Alibre listed on the CAD News Blog HERE. Most of these online 3D symbol libraries would work with either progeCAD (via "acisin" command) or Alibre.
